Rate: 300/hour


120 minutes average length of interview portion (face-to-face)

Record review and report portion - 6-12 hours depending on FAA requirements.


An estimate of final costs will be given to you before scheduling, however depending on findings, additional time might be required for a thorough report. 

Flying In? Traveling from Denver International Airport (DEN) to Fort Collins is straightforward, with several convenient options to suit your preferences and schedule.

Driving

The drive is approximately 70 miles and takes about 50 minutes to 1.5 hours depending on traffic. 

Public Transportation- Denver Airporter

  • Bustang North Line: Board at Denver Union Station (accessible via the A Line light rail from DEN) for a direct bus to Fort Collins. The journey takes about 1 hour and 25 minutes.

  • Groome Transportation: Offers hourly shuttle services between DEN and Fort Collins, with stops at the Harmony Transfer Center and Colorado State University.

  • United Airlines Wingless Flight- book at www.Landline.com or when scheduling your flight, set your destination to Fort Collins/Loveland aka Northern Colorado Regional (airport code FNL).

Fort Collins offers a free city-wide bus service with stops along office route.

Send Records

To ensure a thorough and timely FAA psychiatric evaluation, we kindly request that all relevant medical records be submitted in advance of your appointment. These may include previous psychiatric or psychological evaluations, treatment summaries, hospital records, medication history, and any FAA-related correspondence or documentation.

Records can be securely faxed, mailed, or uploaded via our HIPAA-compliant patient portal. Providing complete and accurate records helps us meet FAA reporting requirements and ensures that your evaluation is as efficient and comprehensive as possible.

What We Provide

Comprehensive Clinical Interview

You’ll meet with a board-certified psychiatrist familiar with FAA protocols. We’ll discuss your medical history, work experience, mental health background, and any recent life changes.

Psychological Testing

Standardized assessments may be used to provide additional data and context. These tools are selected based on FAA guidelines and your specific circumstances.

Records Review

We will request to review prior treatment records, medication history, or related documents to provide a full picture to the FAA.

FAA-Compliant Reporting

Following the evaluation, we prepare a detailed report in the FAA-required format. Our reports aim to be objective, thorough, and submitted in a timely manner.

Recommendations 

If any recommendations are warranted after your evaluation, you will receive a list with explanations provided by Dr. Lubinsky.
